The “Request Extension” page appears.

Clicking this icon available near certain fields takes you to the
help page of the respective field.
The following details are displayed in the header:
Agreement No |
The number identifying the service level agreement raised. |
Revision No |
The number identifying the revision details. The agreement number contains multiple revision number. The combo is loaded with all the previous revision number of the agreement. |
Document Status |
The document status of the service level agreement. |
Start Date |
The date at which the request for extension is started. |
End Date |
The date at which the request for extension is ended. |
Extend Upto |
The date up to which the customer needs to extend the contract. |
Extension At |
Use the drop-down list box to select the level of the extension. The combo is loaded with the values “Line Level” and “Equipment Level”. |
Note: The Line No and Rental Unit is visible only when the “Extension At” is “Equipment Level”.
Line No |
The line number of the service level agreement. |
Rental Unit |
The rental unit against the line number. |

Equipment Code |
The code identifying the equipment. |

The manufacturer and model number of the equipment. |
Ship Date |
The date at which the equipment is shipped. |
Assigned From |
The date from which the unit has been assigned. |
Assigned Till |
The date till which the unit has been assigned. |
Enter the following details:
Extend Upto |
The date till which the contract is extended. |
Reason |
The reason for which the extension is made. |
The system displays the following details:
Extn Acceptance Status |
Indicates whether the extension acceptance status is accepted or rejected. |
Approved Extn Date |
The date at which the extension is approved. |
Click “Extend” button to extend the contract details.
To proceed, carry out the following:
Select the “Attach Notes” link at the bottom of the page to attach the notes information.
The system displays the following in the “Data History” section:
Created By |
The login of the user who created the extension details. |
Modified Date |
The date and time which the extension details were modified. |
