Analyze Failures – A summary of the activity
The failure of any equipment leads to production loss directly or indirectly. The failure may lead to maintenance expenditure to make the equipment working again It may also lead to an accident for the personnel working in the plant or damage to the other machinery in the plant. So the impact of equipment failure can be production loss, expense for maintenance, and safety-related issues. The failure analysis is carried out to rate the impact of the failures of different equipment and/or the impact of different failure modes within equipment. This helps the plant maintenance team in doing a criticality assessment of different equipment and in taking failure management decisions.
Failure analysis can be carried out on the equipment to determine the risk factor of the equipment or the problems that occur on the equipment. A risk factor is computed by the system by multiplying the severity, occurrence and detectability ratings computed for the equipment. This risk factor indicates the impact of failure of the equipment based on the production, maintenance expense and the safety-related issues.
Failure analysis can also be carried out for every problem that occurs on the equipment. This determines the risk factor of problems when they occur on the equipment. A risk factor is computed by the system by multiplying the severity, occurrence and detectability ratings defined for the problems when they occur on the equipment. This risk factor indicates the risk of failure of the equipment, due to the occurrence of the problem on the equipment.
Use this activity to carry out failure analysis on the equipment and on the problems that occur on the equipment. Based on the failure analysis, you can recommend remedial actions that can be taken for reducing the risk factor of the equipment.

The analysis is done based on the severity, occurrence and detectability ratings that have been defined in the “Define Failure Analysis Configuration” activity. A risk factor is calculated based on these ratings and remedial actions are recommended to mitigate the risk factor.
Select the equipment for which failure analysis must be done in the “Select Equipment” page. You can select more than one equipment at the same time.
